Switches control to a fetus that is attached via umbilical cord. The fetus has flight as well as all of Isaac's tear effects and stats.

Effects
- Instead of tears, Lilith launches a baby attached via umbilical cord out of her stomach, dealing x3 damage to enemies it hits.
- The delay between the baby launching is limited by tear delay, so items that increase tear delay like The Inner Eye may make this item much less effective.
- The baby will fire tears while the fire button is held, and moves in the direction of the fire buttons. It returns to Tainted Lilith once fire is released. The baby is not restricted to cardinal directions, and with a gamepad can be controlled with the right analog stick.
- The baby has flight and can only move a limited distance away from Tainted Lilith.
